Inhibition and Facilitation in Cardiac Muscle. Introduction: The effects of subthreshold electrical pulses on the response to subsequent stimulation have been described previously in experimental animal studies as well as in the human heart. In addition, previous studies in cardiac Purkinje fibers have shown that diastolic excitability may decrease after activity (active inhibition) and, to a lesser extent, following subthreshold responses (electrotonic inhibition). However, such dynamic changes in excitability have not been explored in isolated ventricular muscle, and it is uncertain whether similar phenomena may play any role in the activation pal-terns associated with propagation abnormalities in the myocardium. Methods and Results: Experiments were performed in isolated sheep Purkinje fibers and papillary muscles, and in enzymatically dissociated guinea pig ventricular myocytes. In all types of preparations introduction of a conditioning subthreshold pulse between two subthreshold pulses was followed by a transient decay in excitability (electrotonic inhibition). The degree of inhibition was directly related to the amplitude and duration of the conditioning pulse and inversely related to the postconditioning interval. Yet, inhibition could be demonstrated long after (> 1 sec) the end of the conditioning pulse. Electrotonic inhibition was found at all diastolic intervals and did not depend on the presence of a previous action potential. In Purkinje fibers, conditioning action potentials led to active inhibition of subsequent responses. In contrast, in muscle cells, such action potentials had a facilitating effect (active facilitation). Electrotonic inhibition and active facilitation were observed in both sheep ventricular muscle and guinea pig ventricular myocytes. Accordingly, during repetitive stimulation with pulses of barely threshold intensity, we observed: (1) bistability (i.e., with the same stimulating parameters, stimulus: response patterns were either 1:1 or 1:0, depending on previous history), and (2) abrupt transitions between 1:1 and 1:0 (absence of intermediate wenckebach-like patterns). Simulations utilizing an ionic model of cardiac myocytes support the hypothesis that electrotonic inhibition in well-polarized ventricular muscle is the result of partial activation of Ik following subthreshold pulses. On the other hand, active facilitation may be the result of an activity-induced decrease in the conductance of IK1. Conclusion: Diastolic excitability of well-polarized ventricular myocardium may be transiently depressed following local responses and transiently enhanced following action potentials. On the other hand, diastolic excitability decreases during quiescence. Active facilitation and electrotonic inhibition may have an important role in determining the dynamics of excitation of the myocardium in the presence of propagation abnormalities.  相似文献

Summary A ring-shaped electrode was developed and used in 20 patients to record evoked electromyographic responses directly from the extraocular muscles during skull base surgery. Intra-operative monitoring with this electrode helps the surgeon to localize the nerves that innervate the extraocular muscles precisely and to refrain from disturbing important neural structures during operations. Such monitoring also provides some insight into the pathophysiology of the dysfunction of these nerves resulting from skull base lesions.  相似文献

目的:观察蝙蝠葛碱对豚鼠乳头状肌的动作电位是否具有使用依赖性特征,并与奎尼丁、索他洛尔作平行比较.方法:利用细胞内微电极技术记录豚鼠乳头状肌跨膜动作电位.结果:蝙蝠葛碱20μmol·L-1能明显延长乳头状肌跨膜动作电位时程,在刺激周长为200,300,400,600,800,1000,2000ms时,其延长动作电位时程的百分率分别为22±8,11±6,9±5,7±5,6±3,43±28,45±28,蝙蝠葛碱延长动作电位时程作用于刺激周期较短时作用明显,呈使用依赖性特征.而奎尼丁1μmol·L-1和索他洛尔10μmol·L-1延长动作电位时程作用随刺激周期延长而增强,呈现逆向使用依赖性特征.结论:蝙蝠葛碱使用依赖性延长豚鼠乳头状肌动作电位时程.  相似文献

Decorin, a collagen-binding small proteoglycan, is considered to have a specific function in the organization or stability of the collagen network. Therefore, alteration of its molecular properties may be of pathophysiological relevance during the development of cartilage damage. It is shown here that normal cartilage from rabbit knee-joint contains glycosaminoglycan chain-bearing core protein fragments of 39, 23, and 18 kDa, each one amounting to approximately 5-6% of the intact decorin core protein. Continuous infusion of human recombinant interleukin-1 alpha for 14 days (200 ng/day) into a knee-joint led in condylar cartilage to a reduction in the amount of intact core protein from 2 micrograms/mg wet tissue to about 1.1 micrograms/mg. The increase in its quantity found after infusion of heat-inactivated interleukin-1 was not statistically significant. The concentration of all three core protein fragments became reduced to a similar extent as the intact core protein under the influence of the cytokine, and additional fragments were not found. Surprisingly, there was a much smaller response to interleukin-1-treatment in patellar cartilage.  相似文献

使用围套式显微血管减压术治疗面肌痉挛50例。采用耳后斜行小切口,分离压迫血管后,用涤沦片包绕面神经根起始部并用银夹固定,术后49例痉挛停止。本法并发症少,无死亡。术后随访3个月至10年均无复发。根据术中发现将“责任血管”分为4型:接触型、压迫型、粘连包绕型和贯穿型。讨论了手术适应证、要求及并发症的预防等。  相似文献

Summary— Acidosis affects multiple steps in the excitation-contraction coupling pathway of myocardium, producing decreased calcium sensitivity of myofibrils and modification of the function of the sarcoplasmic reticulum. Our aim was to evaluate the effectiveness of three different classes of inotropic agents under acidotic conditions: 1) forskolin, an adenylate cyclase activator that enhances cellular cyclic AMP concentrations, 2) elevated extracellular Ca2+ and 3) endothelin-1, an activator of the inositol triphosphate, diacylglycerol pathway. Ferret papillary muscles were mounted in organ baths containing normal physiological solution (pH = 7.4). After baseline tension was measured, the muscles were bathed in an acidotic solution (pH = 6.98) that decreased tension to 40% of the control; subsequently, the muscles were washed with normal physiological solution until they returned to baseline. Each inotropic agent was added to the bathing solution in a concentration sufficient to increase tension by 40% above the baseline. Then the solution was made acidotic (pH = 6.98) in the continuous presence of that concentration of inotropic agent and the resultant steady-state developed tension measured. The increases in tension induced by each inotropic agent at normal pH were adjusted to be similar; in contrast, the response to each drug in acidosis was significantly different. Under acidotic conditions, endothelin-1 was the most effective inotropic agent in restoring the depressed developed tension. This was possibly due to enhancement of the myofilament sensitivity to Ca2+, which was more effective than increasing [Ca2+]i through elevating extracellular Ca2+ or the addition of forskolin which increased [Ca2+]i but desensitized the myofilaments to Ca2+.  相似文献

背景 针灸是治疗慢性非特异性腰痛(CNSLBP)的重要方法。随着研究的发展,大量针灸治疗CNSLBP的随机对照试验(RCT)已经发表,但由于在结局指标选择方面存在一定的问题,严重限制了高质量临床证据的产生。目的 评价针灸治疗CNSLBP RCT选用的结局指标情况,为针灸治疗CNSLBP相关研究提供参考依据。方法 系统检索4个中文数据库(中国知网、维普网、万方数据知识服务平台、中国生物医学文献服务系统)、3个英文数据库(PubMed、Embase、Cochrane Library)2017-01-01至2022-03-15发布的针灸治疗CNSLBP的文献,以及自建库以来至2022-03-15发表的非特异性腰痛(NSLBP)的核心结局指标集(COS)。对纳入的针灸治疗CNSLBP的文献基本特征进行分析,对文献的临床结局指标进行归类,对文献中的结局指标及《中医病证诊断疗效标准》的应用现状进行分析。分析NSLBP研究的COS现状,比较其与现有针灸治疗CNSLBP文献结局指标的异同。结果 本研究纳入的针灸治疗CNSLBP的49篇文献均为RCT,共包含4篇英文文献和45篇中文文献。共包含4 272...  相似文献

探讨髓芯减压联合血管内皮生长因子(VEGF)与胶原基骨移植材料对兔股骨头缺血坏死的修复作用。方法 对24只SPF级家兔股骨头内注射无水乙醇建立兔股骨头坏死模型,然后将家兔随机分为模型对照组(A组)、髓芯减压+自体松质骨组(B组)、髓芯减压+胶原基骨修复材料组(C组)和髓芯减压+胶原基骨修复材料+VEGF组(D组),每组6只,共治疗12周。通过苏木精伊红(HE)染色观察组织切片形态。采用骨密度分析系统(QCT PRO V6.1)测量家兔股骨头骨密度(BMD)。Western blot检测VEGF、Collagen I、Runt相关转录因子2(RUNX2)、骨钙素(OCN)、Wnt-3a、β-catenin和GSK-3β的蛋白表达。结果 术后12周时,与B、C组比较,D组家兔股骨头骨小梁排列较整齐,骨髓中观察到大量微血管的形成,可见明显成骨,且坏死区基本被修复。与A组相比,B组、C组和D组家兔股骨头空骨陷窝比率均显著降低(P<0.05);D组家兔股骨头空骨陷窝比率小于B组和C组(P<0.05)。与A组相比,B组、C组和D组家兔股骨头的骨密度均显著升高(P<0.05);D组骨密度显著高于B组和C组(P<0.05)。与B组和C组相比,D组VEGF、Collagen I、RUNX2和OCN的蛋白表达水平均显著升高(P<0.05);与B组和C组相比,D组Wnt-3a和β-catenin的蛋白表达水平均显著升高,GSK-3β的蛋白表达水平显著降低(P<0.05)。结论 髓芯减压联合VEGF与胶原基骨移植材料对兔股骨头缺血坏死可有效促进坏死股骨头的修复,提高骨密度及成骨蛋白的表达  相似文献
